‘Fillable Document’ turns your Google document into a live fillable form. These form entries will be stored in your Google spreadsheet. This add-on makes template processing and mail merging easy and visual with Google Docs which comes in handy when you work with templated documents on a day-to-day basis
After you have done so, open the Google Sheets module (e.g. Watch Rows, Search Rows, Get a Cell) and select the Show advanced settings. Then select the Formula option in the Value render option field.  
In order to migrate to a folder within a Shared Drive, this is only possible by specifying a 'Top Level Folder' (Common Settings). This will create a new folder within the Shared Drive and migrate the data into it. It's not currently possible to specify an existing folder within a Shared Drive
